As checked September 21, 2026 (Eastern Time), VA Form 10-10EZR is for Veterans already receiving VA health-care benefits who need to update personal, household financial, insurance, or military-service-history information. It is not the initial enrollment application; use VA Form 10-10EZ for that. VA offers online, phone, mail, and in-person update routes, but its current instructions say military service history can be updated only with the 10-10EZR PDF at this time. An income update is not an annual requirement for every enrolled Veteran. Reporting a change may affect copays, prescriptions, travel-pay eligibility, or a Priority Group only after VA reviews the individual record. Start at VA’s current update instructions and 10-10EZR form page; send sensitive information only through an official VA channel.
Is 10-10EZR the form you need?
The distinction is already receiving VA health care versus applying to enroll. VA describes Form 10-10EZR as the Health Benefits Update Form for a Veteran who already receives VA health-care benefits. The 10-10EZ form page and application instructions govern an initial health-care application. If you are uncertain whether you are enrolled, ask VA’s Health Eligibility Center before choosing a form. The 10-10EZR updates information VA uses to administer your own VA health-care benefits. It does not by itself apply for disability compensation, add a dependent to a compensation or pension award, enroll a family member in CHAMPVA, or correct a Department of Defense service record. Use the task-specific route in the table below.

How can you update your VA health-benefits information?
VA’s current instructions offer four channels. Choose the one that fits the information being changed, and review the current VA page before sending a completed form:
- Online: Start from the official 10-10EZR online tool. Its public introduction is visible without signing in; completing an update requires a VA.gov sign-in. Do not assume every field on the paper form is available online.
- Phone: Call VA’s Health Eligibility Center at 877-222-8387, select 1 (TTY: 711). VA currently lists Monday through Friday, 8:00 a.m. to 8:00 p.m. Eastern. Ask the representative whether your particular change can be completed by phone.
- Mail: Download the current five-page 10-10EZR PDF, read its two pages of instructions, complete the applicable sections, and sign and date it. VA’s current address is Health Eligibility Center, PO Box 5207, Janesville, WI 53547-5207. Include supporting material only when appropriate for your update; send it to VA, not to this website.
- In person: Ask for an enrollment specialist at a VA health facility. The current VA update page expressly allows an in-person update.
VA’s form page labels the current 10-10EZR revision February 2025. The PDF itself shows that revision on all five pages. Its first two pages explain which sections apply and how to sign, including the Power of Attorney and witnessed “X” signature branches. Use the latest PDF linked from VA’s form page if the revision changes. This article is an explanation, not a substitute form or submission portal.
Before mailing, check that you have the revision currently linked by VA, that every applicable section is complete, and that the person authorized to sign has signed and dated Section VIII. The PDF says an authorized Power of Attorney signer must include the Power of Attorney document; an “X” signature needs two witnesses who sign and print their names. Attach a continuation sheet if the form calls for one and any optional supporting material you choose to provide. Keep a copy for your own records. If you need confirmation that VA received or processed an update, ask the Health Eligibility Center or your VA facility; this guide cannot see your VA record or promise a processing time.
Why does the service-history route need extra care?
The current VA update page specifically says that, at this time, military service history can be updated only with the PDF version of 10-10EZR. VA’s public online-tool introduction broadly lists military-service-history information among potential updates, but its actual signed-in fields are not public. For a service-history change, follow the specific PDF instruction or confirm the current route with VA’s Health Eligibility Center. This is a current channel limitation, not a permanent rule about future online tools.
Which information belongs on the update?
The official update guidance and current PDF describe these areas:
- Personal and contact information: name, mailing and home address, phone, optional email, marital status, emergency contact, and next of kin. A profile change may update several VA services, but VA warns that some programs keep separate records. Check the health-benefits record and any other relevant program separately.
- Health insurance: every plan that covers you, including coverage through a spouse or other person, an employer or private plan, Medicare, and Medicaid. The PDF asks for insurer, policyholder and policy details, plus Medicare and Medicaid information. Use the form’s separate-sheet instruction if more insurance information is needed.
- Household and finances: marital status, relevant spouse and dependent details, the previous calendar year’s gross income for you and applicable household members, and permitted deductible expenses. The February 2025 PDF’s Section V covers prior-year gross income; Section VI covers deductible expenses. Its instructions say to complete the dependent-information Section IV only if you complete the financial Section V. Follow the form’s own definitions and the VA representative’s guidance for an unusual household situation.
- Military service history: service dates, discharge and service details, Purple Heart and other service-history questions, and toxic-exposure or hazard information. The PDF’s Section III has the service and exposure questions. Reporting a history lets VA review it; a checked box, DD214, or statement does not automatically prove exposure, establish a disability claim, or award a Priority Group.
The February 2025 PDF says all Veterans completing it must fill Sections I, II, VII and VIII; its other sections depend on the update. Section VIII requires the proper signature and date. The PDF may ask for sensitive identifiers and financial, insurance, or service information. Enter those only in VA’s official tool or form, or provide them directly to VA through its official channel. WVCOFCSRA does not collect, receive, or store any 10-10EZR information.
For a paper financial update, read the PDF’s definitions before entering totals. Section V asks for previous calendar year income, and its instructions distinguish gross employment income, net farm or business income, and other amounts such as retirement, pension, Social Security, compensation, interest, and dividends. The instructions also name amounts not to report, including Supplemental Security Income and certain need-based payments. Tax treatment alone does not tell you how the VA form classifies an amount. Section VI asks for eligible non-reimbursed expenses; the PDF names medical and dental expenses, final illness and burial costs, and certain college or vocational education expenses. Do not include a cost you expect another source to reimburse. When a household or income definition is unclear, use the form’s instructions or ask VA instead of estimating from this guide.
Are supporting documents required for a service-history update?
VA says no supporting document is required just to provide exposure or service-history information, although material may help VA confirm details. Its examples include DD214 or DD215 separation papers, other service records, orders or unit histories, a personal or buddy statement, and relevant photos or contemporaneous articles. If you submit material, describe the location, period, hazard and activity as accurately as you can. Do not treat any one record or statement as automatic proof. The current PDF also allows continuation sheets for information that does not fit.
Must every enrolled Veteran update income every year?
No. VA’s health-care costs guidance gives different branches. A Veteran who completed a financial assessment for cost-free medications or beneficiary travel, but not for free VA health care, must provide updated income information each year. A Veteran whom VA found eligible for free VA health care because household income was below its limit does not have to submit updated income each year solely for that reason; VA says it checks IRS and Social Security Administration information and contacts the Veteran if a change may affect eligibility or copays. VA still encourages reporting income changes even when an annual update is not required.
Other Veterans may qualify for a level of care based on a service-connected rating, pension, recent combat service, qualifying exposures or another rule, and VA explains when income information may help assess other benefits. If you do not know why VA requested a financial update, ask the Health Eligibility Center which branch applies to your record. The online tool’s public introduction currently refers to 2025 income and expenses because those are the preceding calendar year in 2026; use the year the current VA tool or form requests when you actually update.
An accurate change may lead VA to review copays for non-service-connected care, prescription costs, beneficiary travel eligibility, or Priority Group. VA explains the cost branches and Priority Group factors. Reporting an amount does not promise a particular group, exemption, refund or travel payment; VA determines the result from the complete individual record.
The financial assessment is about what VA needs to decide health-care costs or related eligibility. A changed household does not, by itself, establish that VA should add someone to a compensation award. Likewise, receiving a VA request for updated information does not prove that an annual submission rule applies to all Veterans. Read the request for the specific record and period, and ask VA to identify the branch if it is not clear.
What happens when you report other insurance?
Tell VA about insurance that covers you, including spouse-provided or employer coverage. VA says other insurance does not itself change the VA health-care benefits you can receive. VA must generally bill private insurance for care, supplies or medicines for non-service-connected conditions. It does not bill Medicare or Medicaid, though it may bill a Medicare supplemental plan for covered services. Private-insurer payments may offset some or all of a VA copay, but the offset is not guaranteed. Whether a VA copay applies remains a separate VA determination.
VA health care and Medicare or a private plan can serve different settings and family members. Reporting coverage is not an instruction to cancel another plan. If the insurance belongs to a CHAMPVA beneficiary rather than the Veteran’s own VA health-care record, follow CHAMPVA’s separate Form 10-7959C and its instructions. Which changes need another VA process?
Changing a dependent for the Veteran’s health-care financial assessment does not submit a claim to add that person to disability compensation, pension, or DIC. VA’s dependent-benefit page and Form 21-686c describe the separate request. Likewise, VA.gov profile updates can change contact details across several programs, but some program offices require direct contact. A profile entry does not replace a 10-10EZR financial or insurance update.
The service-history section helps VA administer health-care eligibility and possible exposure-related Priority Group decisions. It is not a disability-compensation claim; use VA’s disability filing process if that is your task. It also does not directly correct military information in the Defense Enrollment Eligibility Reporting System or obtain a corrected military record. VA’s profile guidance directs incorrect military-profile data to the Defense Manpower Data Center, and VA’s military-records page describes record requests. Ask the responsible agency which correction process applies to the actual record.
Safe next step
Check VA’s update page and form page immediately before acting, especially for the PDF revision, online scope, phone hours or mailing address. If your situation spans more than one VA program, identify each record separately and ask the responsible VA office which transaction it needs.
